Incident Report for Coaches & Volunteers
This is the form that must be filled out in the case that a Manager, Coach, Umpire or other volunteer is injured on a Palatine Park District field or park.

Incident Report for Players or Spectators
This is the form that must be filled out in the case that a player or spectator is injured on a Palatine Park District field or park.
